- RN Wellness Coordinator
Description
Primary to the role is ensuring quality resident care and service is provided in a manner that upholds the Prelude philosophy and well-being of the residents. Responsible for demonstrating a sincere desire to work with elders with memory loss, demonstrated through a loving and kind approach and attitude.
This position works in partnership with the Director of Clinical Services, the Licensed Assisted Living Director, LPN Campus Wellness Coordinators.
Requirements
Reports to: Director of Clinical Services
Clinical:
- As needed and directed participate in resident care conferences
- Complete preadmission assessments as needed.
- As needed, assist with the move-in process and transition of new residents into the cottage – spend time getting to know their routine; assist staff with interventions as needed.
- Complete all required resident assessments
- When completing ongoing assessments, review charting of care and activities ensuring they’re completed and consistent with the needs of residents. Communicate with Campus LPN Wellness Coordinator in areas of concern.
- In partnership with Campus LPN Wellness Coordinator and Licensed Assisted Living Director work with staff to promote activities as an integral part of daily life
- In partnership with the Campus LPN Wellness Coordinator and Licensed Assisted Living Director meet as needed with residents, families and staff, formally and informally to assure the delivery of services and support is provided and addresses concerns and areas of opportunity.
- As needed as a Bluestone liaison
- Participate as directed with Vulnerable Adult Investigations and reports (along with LPN Campus Wellness Coordinator, Licensed Assisted Living Director and Director of Clinical Services)
- As needed Communicate with MD's regarding resident needs
- As needed, we assist with processing new orders.
- Review incidents, accidents, or Med error reports. Investigation as needed
- Report incidents of suspected resident maltreatment (abuse, neglect) to the Director of Clinical Services and Licensed Assisted Living Director. Assist the Director of Clinical Services in reporting the findings to the appropriate person or agency.
- Participate in nursing on-call rotation
Resident Care:
- As needed Assist with resident care. Typically, and generally in an emergency.
- Help each resident reach and maintain the highest level of performance and independence.
- As needed Complete treatments according to the residents’ individualized care plan.
- Observe resident and report changes in physical and emotional condition such as change in attitude, appearance, loss of appetite, weight, etc. initiate reassessment and service plan if needed
- Assist with the move-in process and transition of new residents into the cottage – spend time getting to know their routine; assists staff with interventions as needed.
- In partnership with Prelude Ministries ensure completion of the Life Legacy and that personal preferences are incorporated into the daily routine of each resident
- In partnership with the site Wellness Coordinator and Director Clinical Services schedule and participate in resident care conferences
- In partnership with the site Wellness Coordinator and Director of Clinical Services ensure resident care plans including physical, social, and personal needs are met
Staff Supervision:
- Ensure staff is compliant with Prelude’s mission and philosophy of care and service.
- Ensure staff is aware of and comply with policies and procedures established by the organization.
- Participation as needed with the interviews and selection of new team members
- Participate in training as assigned.
- Provide ongoing mentoring, training, and coaching of team members.
- Lead and mentor the best practice of Assisted Living and Dementia care.
- In partnership with the staffing team ensure 24-hour coverage of the Cottages
- In partnership with the staffing team, manage overtime and agency usage.
- Supervise, coach and document performance reviews, coaching and disciplinary matters.
- Review and approve payroll, PTO requests and all variances from the schedule
- In partnership with fellow LALDs, Director of Training and Development and Director of Operations conduct monthly Caregiver meetings
- Round each Cottage multiple times per day
- Accountable for staff communication
Operations:
- Serve as a member of the Prelude leadership team.
- In partnership with the Assisted Living Director, the Director of Clinical Services and LPN Wellness Coordinator identify and oversee the correction of any health or safety issues.
- In coordination with the Director of Clinical Services, the Licensed Assisted Living Director and Campus LPN Wellness Coordinator identify when a resident is having difficulty meeting the residency requirements or when the resident’s needs are likely to exceed the services available from Prelude staff.
- Maintains positive relationships with residents, their families, housing and home care staff and the larger community
- Promote and support Prelude’s operational pillars, including spiritual care, music, Life Legacy and meaningful engagement.

Education, Experience, Certification, Licenses
- Minimum Must be registered and licensed with the State of Minnesota as a Registered Nurse, and must maintain current license
- Experience working with the elderly, specifically those with Dementia, strongly preferred
- Previous home care or public health nursing experience is preferred and beneficial.
Language Skills:
Ability to read and comprehend simple instructions, correspondence, and memos. Ability to write simple correspondence and documentation for each resident. Ability to effectively present information on one-to-one and small group situations to customers, residents, and other employees of the organization.
Working Conditions:
- Primary responsibilities will be conducted in clean, well-lit, climate-controlled areas such as resident rooms, hallways, work rooms, resident dining areas, and meeting rooms.
- The noise is usually moderate.
- Exposure to chemical and respiratory hazards.
- Some travel to meetings and seminars may be required.
- Exposed to body fluids, infection, odors and behavior of residents.
Physical Demands:
- Must be able to move throughout the workday and throughout the building.
- Must have adequate vision and hearing to meet the requirements of this position.
- Must be able to cope with the mental and emotional stress of the position.
- While performing duties of this job, employees are regularly required to use hands and fingers to examine residents, handle or feel objects, tools, or controls, and reach with hands and arms.
